Job Description
The Homecare Referrals Coordinator is responsible for the smooth transition of the patient into the home health environment. Receives and coordinates referrals to Lahey Health at Home from affiliated and outside physicians, hospitals, facilities and other community referral sources
Qualifications
Registered Nurse License
1-2 years experience in Home Health Care.
Comprehensive knowledge of home health care.
Demonstrates excellent observation and problem solving/critical thinking skills.
Well-developed verbal and written communication skills; essential computer skills to work with EMR.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Self-directed and able to work with minimal supervision.
Shows strong ability to organize and prioritize workload independently.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
